Somerset County Seeks Input for Senior Needs Assessment Survey

The Office on Aging and Disability conducts a survey annually to determine the needs of the aging community and people with disabilities.

The Somerset County Office on Aging and Disability Services needs your help in planning necessary programs and services for people aged 60 and over, caregivers, and people, 18 years of age and above, living in the community with physical disabilities.

“We are asking residents to complete this survey to guide us in making decisions about the most needed community-based services in Somerset County,” said Commissioner Elizabeth Graner, liaison to Somerset County’s Human Services Department. “Our goal is to gain a better understanding of how we can serve you and be as responsive to community needs as possible. This is a great opportunity to leverage your voice and help shape long-term programming within Somerset County.”

In compliance with the Older Americans Act of 1965, the Office on Aging and Disability conducts a survey annually to determine the needs of the aging community and people with disabilities. The Office on Aging and Disability includes survey results in the Service Area Plan, which is developed every three years. The Area Plan helps to identify needs and trends to implement programs and services to address those needs.

All information is kept confidential. If there is more than one eligible member of the household, each person should complete their own survey. Caregivers may complete the survey for a loved one who is unable to complete the form.

The Somerset County Office on Aging and Disability Services serves as an Aging and Disability Resource Connection (ADRC) and is a hub for long-term care support services, health promotion, and education. The agency is committed to promoting health, independence, dignity, and choice for seniors as well as adults with physical disabilities, and caregivers in Somerset County. There is a wide range of services and programs that are currently funded, which include information and assistance, home health services, Project Lifesaver for individuals with dementia, congregate nutrition programs, home delivered meals and wellness programs.
